REGISTRATION INFORMATION

Online registration will be available on our homepage on the left hand side under the Registration tab beginning on May 1^st. It is highly recommended that you register your child online as soon as possible.

If you are unable to register online, you may print a registration document which can be found on the website located under the Documents tab on the left hand side of the homepage beginning on May 1st.

We will be hosting a walk-in registration/information night on Thursday, May 13, 2010, from 5-8pm, at Faith Mountain Church. GMJFA Board members will be available to help with registration and answer any questions about the upcoming season. Please click here for a map to Faith Mountain Church.

ADDITIONAL DOCUMENTS REQUIRED FOR REGISTRATION:

Photocopy of Player’s Birth Certificate/Passport

Returning players from GMJFA last year do not need to provide this information.

Photocopy of Player’s School Report Card/Registration

All players (new or returning) need to provide this document.

Proof of Residency (MUST be Comcast, Qwest or Xcel Energy or Water bill. No other documents will be accepted per JMFA Rules.)

Returning players from GMJFA last year who reside at the same address as last year do not need to provide this information.

Physical

Physicals must be completed and signed by a MD, DO, NP or PA. The medical form can be found under the Document tab on the left hand side of the homepage. Physicals must be completed and turned in prior to the first practice; otherwise players will not be allowed to participate until completion. Physicals completed after February 10, 2010 are acceptable for use for the 2010 football season.
